New Patient Registration

non_nhs_services1The doctors welcome new patients who live within our practice area. In order to register with the practice you should bring your NHS medical card. If you don’t have a medical card a receptionist will ask you to fill in a registration form.

Patients are registered with the practice, not an individual GP.

New Patient Screening

All new patients are requested to make an appointment for a general health check with a health care assistant when they register with the surgery. These checks assist the doctor as they provide information concerning your general state of health prior to the arrival of your medical records. We ask you to confirm your attendance 48 hours before your appointment. Appointments not confirmed will be cancelled. (Please bring a urine sample).

Registration Form

You may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
